24 hour care

Alzheimer's and Dementia Care

Transition Care

If you have been in hospital and are ready to return home, but need some home help and personal assistance to support your recovery. Experiencing difficulty with simple activities of daily living or facing unforeseen challenges around the home, other issues such as mistakes made in medications, missed follow-up medical appointments, poor nutrition, dehydration or lack of safety preparation in the home. 

Our Occupational Therapist will provide pre- & post-hospital home visit to ensure safety and soothing the recovery process. 

Our Carers will provide assist with daily tasks and according to your hospital discharge planning to develop a detailed plan of care that will be used by family and other carers to ensure a successful recovery.

Our carers will help you organise and keep track of your medication to ensure that they are taken as directed. They will also help you avoid any adverse effects or monitor any reactions you may experience. Our carers are trained to record specific information on your post-discharge care and recovery. Keeping a record of your transition care and recovery progress is important to share with your doctor and can be used to update. 

Often, after a visit to hospital, we know it is not easy to cook your own meal, we can arrange food delivery from your favorite restaurants, our carers can also assist you with meal planning, shopping, preparation and cooking.  

Our Carers will help you keep track of all of your follow-up medical appointments. A carer can personally drive and escort you to your appointments or arrange transport for you.

 

Companionship
